Clicking on the Achievements and Awards Report Form button below will open the Achievements and Awards PDF file, which has numerical entry fields or drop-down fields for all items. Select the Quarter for the report, select your Club, then complete all of the items in the form that apply for your Club for the report quarter.

When you are done, click on the Print button to print a copy of the form. You can then click on the Save Form to save the form on your computer, then e-mail it to the Achievements and Awards Chairperson Stan White. His e-mail address and mailing address are both shown on the form.
